Primary Care Clinic Form

The Reproductive Health and Wellness Program services all patients, male and female. Services are provided in regards to reproductive health.

Birth control, pregnancy education, sexually transmitted infection testing, condoms, and any other reproductive related services can be covered in this program. Medicaid and many private insurances can be covered.

In accordance with Title X, a sliding scale program exists for those without insurance, or unable to use their insurance, to discount the fee rates. This can go down to 100% INDIGENT, where the client may owe no payment for services. Services are confidential.

Age 14 years and older can be seen for these services without parental consent under Title X. Family participation and abstinence are encouraged. See the sliding fee scale document attached to website, to see where you would fall on the scale for discount.

If unable to pay even with discount call or come to office and we can assist with this. NO PERSON will be turned away FROM THE REPRODUCTIVE PROGRAM due to inability to pay. This sliding scale and waiver of all fees is only possible with REPRODUCTIVE SERVICES, and does not apply to the primary care clinic or cash fee schedule for primary care.
